The benefit materials, policies, and applications available on this website are currently reflective of the AY2022 plan year. We are working expeditiously to have the AY2023 information updated as soon as possible.
For now, students can view a list of AY2023 changes.
 

Cancellation

A student may request a cancellation of dental coverage for themselves and/or their dependent(s) through Member Services. This request will only cancel the dental plan in which you enrolled; it will not cancel the Student Health Fee or the Student Health Insurance Plan. The date we receive the cancellation form will determine their cancellation options. View cancellation policy and application

Change Plan Type

A student may request to switch dental plan enrollment options within 5 business days of their application submission date. Changes in plan types will apply to all family members enrolled on the student’s dental plan. View change plan type policy and application
