Dental Blue Cancellation Policy and Application

You may be eligible to cancel the optional Dental Blue insurance if you meet one of the criteria below:

  • No services were used for the term in which you are requesting to cancel.
    AND ONE OF THE FOLLOWING CONDITIONS ARE MET:
  • The cancellation application is received within five days of submitting the enrollment application. Coverage will be canceled retroactively to the plan effective date for the term you are enrolled. The cost for the plan will be credited in full to the student’s account.
    OR
  • The cancellation application is received by the enrollment deadline.
    • Enrollment Deadlines:
      Fall term: September 30
      Spring term (must meet criteria): February 28
  • Coverage will be canceled retroactively to the start date of the term in which you are enrolled (i.e., 8/1 for the fall term or 2/1 for the spring term). The cost for the plan will be credited in full to the student’s account. If you are enrolled for the fall term (full-year plan), you are not eligible to cancel spring term only coverage.     OR
  • If you (or any enrolled dependents) gain other insurance, coverage can be cancelled retroactively 45-days from the effective date of obtaining new dental insurance. Note: that the 45-day cancellation policy is specific to the effective date of the coverage and not when the member enrolled or signed up for coverage.
  • This means that you must submit your cancellation request with an official coverage letter within 45-days from the effective/start date of the new insurance effective date.
  • Coverage will be cancelled retroactively to the effective date of the other coverage so long as the cancellation application and documentation (proof of your new coverage) are received within 45-days of the start date (effective date) of your new dental coverage. For example, if your new dental insurance effective/start date is October 1, the deadline to submit your cancellation form with proof of another enrollment is November 15. Cancellation requests received after 45-days will not be considered.
  • Appropriate documentation to support the event is mandatory and must be submitted with your cancellation form.
    • You must provide a letter from the other employer or insurer indicating that you and/or enrolled dependents are enrolled/approved for other dental coverage. The document must have the name(s) of those enrolled and the effective date of the coverage. The cost will be prorated to the date the coverage is canceled. You cannot re-enroll until the next open enrollment period or within 60 days of losing other dental insurance coverage.
  • Coverage will be canceled retroactively to the plan effective date (i.e., 8/1 for the fall term or 2/1 for the spring term) if canceled by the enrollment deadline or within five days of submitting the enrollment application.
  • Coverage will be canceled retroactively to the effective date of the alternative dental plan if canceling within 45 days of gaining other dental coverage. The cost will be prorated to the date the coverage is canceled.
  • If applicable, a credit will be applied to your student account within 10-20 business days.
  • Re-enrollment will be available during the next open enrollment period in which you are eligible to enroll or within 60 days of losing other dental insurance coverage (documentation required).
  • Appeals to this policy will not be considered.
