Visit https://provider.bcbs.com/ to search for an in-network specialist nationwide
Enter your address, city, or zip code
Type “HVA” as your three-letter prefix OR select BlueCard PPO/EPO as your plan

Sample Out-of-Pocket Costs for Specialist Care

Clinic and doctors office visits Diagnostic/specialist/sick – adult 18 and older	$35 copayment	30% co-insurance (after deductible is met)

Note: Not all BCBS providers are in the BCBS PPO network. If you want the lowest out-of-pocket costs, be sure your specialist is in the BCBS PPO network.
